How to Complete USPS Form 1583 for a Virtual Mailbox
A plain-English walkthrough of the federal form that has to be on file before any mail-receiving agency can accept mail on your behalf.
What Form 1583 actually is
PS Form 1583 is titled Application for Delivery of Mail Through Agent. It is the document that authorizes a Commercial Mail Receiving Agency — a CMRA — to receive and handle mail addressed to you. It is required under USPS regulation, not provider preference, so every legitimate mailbox service will ask for it.
The form exists so the Postal Service can identify who is using a given private mailbox. That is also why the identity requirements are stricter than most people expect going in.
The current version is the April 2023 revision. Check the bottom of the page you downloaded — it should read PS Form 1583, April 2023. Download it directly from usps.com rather than a third-party copy, since an outdated version is a common reason applications come back.
What you need before you start
- Two forms of identification. At least one must be a government-issued photo ID, such as a driver's license or passport. The second has to verify your current address — commonly a lease, vehicle registration, or insurance policy. The form lists what qualifies.
- Your real physical address. This is your actual home or business address, not the mailbox address you are applying for. Using the mailbox address here is one of the most frequent errors.
- Your legal name exactly as it appears on your photo ID. Nicknames and shortened forms cause rejections.
- For a business: the entity name, and the name of each person authorized to receive mail for it. Some agencies also ask for proof the business exists, such as articles of incorporation or a business license.
- A notary, in most cases. If you are not completing the form in person, it generally has to be signed before a commissioned notary. Online notarization is widely accepted and usually takes minutes.
Things that surprise people
Every adult needs their own form
If more than one adult will receive mail at the same box — business partners, spouses — each person completes a separate Form 1583. One form does not cover a household or a team.
The agency keeps your documents on file
Your completed form and ID copies stay with the CMRA. That is a USPS requirement, and it is part of why the process is more involved than renting a P.O. Box.
It does not expire
Once filed and accepted, Form 1583 stays valid for as long as you keep using that same agency. Moving to a different provider means starting over.
A virtual business address is not a registered agent
These get conflated constantly. A registered agent receives legal and state correspondence under specific state rules. A virtual business address is a mail-handling service. Some businesses use both. They are not substitutes for each other, and whether a given address satisfies a particular filing, bank, or licensing requirement depends on the rules of whoever is receiving it. Before you rely on any address
Form 1583 authorizes mail delivery. It does not by itself determine whether an address is acceptable for a state business filing, a bank account application, a professional license, or a tax document. Those requirements are set by the agency or institution asking for the address, and they vary.
